A Canadian Healthcare organization is a community-based organization providing healthcare services across the region. The organization is leading a Program which is a collaborative effort between several partner organization. The initiative aims to improve outcomes for a targeted population in the regional community by enhancing coordination, service delivery, and digital connectivity between partner organizations.

Main Objectives

The goal of the engagement was to develop an Information and Digital Strategy to support for a regional health equity initiative Hub. Specifically, AIM was tasked to:

• Assess the current IT landscape across all partner organizations.

• Identify opportunities for integration, modernization, and shared digital infrastructure.

• Recommend a technology roadmap and governance model that enables secure data sharing, improved client experience, and operational efficiency.

• Provide a procurement and implementation plan for unified systems supporting service coordination and reporting.

• AIM’s engagement was structured into six phases, as defined in the signed agreement and project schedule:

• Requirements Review & Documentation – Collect and validate business and IT requirements across the three partner organizations.

• Systems Review & Gap Analysis – Conduct a detailed IT assessment to identify overlaps, risks, and modernization opportunities.

• Market Scan & Solution Options – Evaluate potential technology platforms (CRM, case management, and data integration tools).

• Solution Selection Process – Facilitate stakeholder alignment and recommend the most suitable solution stack.

• Implementation & Procurement Planning – Develop a high-level rollout roadmap, procurement strategy, and governance plan.

• Implementation Oversight – Support the client organization’s execution team with technical guidance and validation of outcomes.

Technologies and Tools

AIM’s multidisciplinary team leveraged its digital transformation framework based on best practices from Gartner’s TIME model, NIST SP 800-34, COBIT, and ITIL standards. The engagement included analysis of the client organization’s Azure-based cloud infrastructure, on-prem systems, and third-party tools used by the partner organizations. Where relevant, AIM used the IT Health Check methodology previously applied for another healthcare organization and criticality assessments used in the client organization’s own IT Service Recovery Planning.

Results / Expected Outcomes

• A unified digital roadmap for all the partner organizations that supports integrated service delivery under the regional health equity initiative model.

• Identification of a shared IT governance framework for coordinated decision-making and long-term sustainability.

• Improved data interoperability and privacy alignment across organizations.

• A clear procurement and implementation roadmap that guides future investment decisions.

• Enhanced readiness for cloud adoption and cybersecurity resilience, informed by AIM’s NIST-based assessment methodology.

• These outcomes are expected to strengthen the partners’ collective ability to deliver equitable, accessible, and data-driven health services to the target population in the regional community.

Conclusion

Through its partnership with a leading community healthcare organization, AIM is helping establish a modern, secure, and collaborative IT environment that will serve as the digital foundation for the regional health equity initiative Hub. This engagement reflects AIM’s deep expertise in healthcare digital strategy, IT governance, and multi-partner system integration, building on prior successful projects with another healthcare organization, a national healthcare provider, and a large nonprofit organization.
